			<content:encoded><![CDATA[<p>Even though wavefront-optimized LASIK has produced excellent visual acuity for patients, U.S. Army ophthalmologists are investigating whether phakic intraocular lenses might be better option for myopic soldiers, given their special visual needs, particularly at night.</p>
<p>Phakic IOLs are known to have minimal impact on higher-order aberrations of the optical wavefront. Might this characteristic give soldiers with phakic IOLs a visual advantage in combat, compared to wavefront-optimized LASIK?<br />
U.S. Army Maj. Gregory D. Parkhurst, MD, chief of refractive surgery at the Darnall Army Medical Center at Fort Hood, Tex., has been investigating this question. He says early hints suggest the answer might be yes.</p>
<p>(Dr. Parkhurst agreed to talk about his early results, but he emphasized in his interview with Refractive Surgery Outlook that he was giving his personal opinions as a physician, and was not speaking for the military.)</p>
<p>&#8220;Today, excimer lasers with wavefront-corrected ablation profiles produce excellent visual acuity for patients. We&#8217;re hitting 20/20 post-op acuities almost all the time. So now the issue is quality of vision, an area where phakic IOLs excel,&#8221; said U.S. Army Maj. Gregory D. Parkhurst, MD, chief of refractive surgery at the Darnall Army Medical Center at Fort Hood, Tex.</p>
<p><strong>Early Verdict: Safe for Soldiers</strong></p>
<p>Based on a retrospective study of results in 104 active-duty military personnel (206 eyes), Dr. Parkhurst concluded earlier this year that the Visian Implantable Collamer Lens (ICL, Staar) is a safe, predictable refractive surgery option for military personnel who are not candidates for laser vision correction.</p>
<p>In the fall, he hopes to present the results of a Fort Hood study that tested soldiers&#8217; visual acuities with a tool that mimics the low luminance of &#8220;night vision goggles&#8221; (Rabin Super Vision Test, Premiere Vision). Although he could not give specific details, Dr. Parkhurst spoke generally about the prospective study&#8217;s results.</p>
<p>In 25 carefully matched pairs of patients (pre-op sphere: -3D to -9D), a preliminary analysis suggests that the subjects with bilateral ICLs had better low-contrast UCVA than patients who had undergone wavefront-optimized LASIK (Allegretto Wave Eye-Q Excimer Laser, Alcon). This appeared to be the case with and without the simulated night vision goggles.</p>
<p>The differences in low-contrast acuity were small—approximately two to three letters on a logMAR chart. They were detectable and statistically significant, but their visual significance to soldiers on the battle field and other professionals requiring excellent quality of vision, such as police officers and professional athletes, remains to be shown, he said.</p>
<p><strong>Small Differences in HOAs</strong></p>
<p>&#8220;These are very small differences in HOAs between the ICL and LASIK eyes,&#8221; Dr. Parkhurst said. &#8220;The highest bar on my chart shows 0.45 microns of induced higher- order aberrations. That is a really low number. For example, in LASIK we don&#8217;t usually consider doing a custom ablation on a patient if pre-op HOAs are 0.4 microns or less.</p>
<p>&#8220;There&#8217;s something about the quality of vision with this lens that is much better than laser vision correction,&#8221; Dr. Parkhurst said. &#8220;Due to word of mouth we&#8217;re getting to the point that soldiers are now asking for ICLs as their first option, even when their corneas have no extra risk factors from LASIK or PRK. An OR nurse last week told me he was approached when he was out shopping at the commissary and asked about how to get on the list for a collamer lens.&#8221;<br />
Dr. Parkhurst and other Army refractive surgeons at bases around the country are discussing the possibility of larger, multicenter clinical trials to investigate the quality of vision with ICLs further.</p>
<p><strong>Abundance of Caution</strong></p>
<p>&#8220;We&#8217;re going slowly, trying not to be too aggressive,&#8221; he said. &#8220;The next study needs to be a randomized, multicenter, controlled study in patients with normal eyes that could have LASIK if they chose. We need this to reassure us that if someone gets an ICL instead of LASIK, they&#8217;re not subject to greater complications.&#8221;</p>
<p>Dr. Parkhurst&#8217;s department is on pace this year to perform about 6,000 refractive surgeries on military personnel for whom lost or broken eyeglasses can be a life-or- death situation.</p>
<p>Concerns about flap displacement during combat have made PRK the most common procedure (60 percent). About 25 percent have LASIK and the rest opt for a single type of phakic IOL, the Visian ICL, because of its small incision size and position in the posterior chamber, he said.</p>
<p><strong>Key Concern: Globe Trauma</strong></p>
<p>The U.S. Army has been cautious about embracing phakic IOLs for its soldiers out of concern that the lens might not be stable or safe during combat trauma. Then in 2008, McCauley et al. reported on a soldier whose phakic IOL remained stable despite a grenade attack, orbital shrapnel and blunt globe trauma. [1]</p>
<p>&#8220;That case was considered evidence that the ICL would be stable despite traumatic globe injuries,&#8221; Dr. Parkhurst said. &#8220;So the Army okayed a program of using the ICL in candidates with thin corneas or other contraindications to laser vision correction. Our program at Fort Hood was one of the first.&#8221;</p>
<p>His department&#8217;s initial study on 206 eyes of 104 patients found that 100 percent of eyes with three months follow-up (n=139) were within 1.0 D of the refractive target; 89 percent were within 0.5 D.
<p>
<strong>ICL Safety in 104 Patients</strong></p>
<p>UCVA was at least 20/15 in 67percent of patients and at least 20/20 in 96 percent (with the remaining refractive error caused by uncorrected astigmatism). Preoperative sphere was between -2.50 D and -11.00 D (mean -5.86; SD 2.92). BCVA declined by one line in a single eye; the patient subsequently was referred for investigation of nyctalopia and possible retinal dystrophy.</p>
<p>Among eyes with nearly two years follow-up, no incidence of cataract or any other significant complication has been observed, Dr. Parkhurst said.</p>
<p>&#8220;We do still worry, though, about the longer term risk of cataract and of endothelial cell loss because these patients are mostly very young,&#8221; he said. &#8220;The FDA age frame for the ICL is 21 to 45, and in this study the average age was 30.&#8221;<br />
Complications included: 4.8 percent occasional glare or haloes, none causing functional impairment; one case of iritis; one case of nyctalopia; and three explants (one for excessive vault, two for wrong IOL power.</p>
<p>Dr. Parkhurst said he believes these initial results support use of the ICL, at minimum, to correct myopia in the 10 to 12 percent of soldiers he sees who are not candidates for excimer laser correction. But he expects more and more patients with lower levels of refractive error to choose the ICL, especially after a toric version becomes available.</p>
<p>&#8220;We&#8217;re in an environment where out-of-pocket cost to the patient is not an issue. The active-duty personnel who come in here pay for their surgery with service to their country,&#8221; he said. &#8220;There&#8217;s no financial incentive here to use one technology over another. A surprising number of them choose the ICL, because they like the idea of a technology that&#8217;s more reversible.&#8221;</p>

			<content:encoded><![CDATA[<p>Dr Daniel Goldberg, one of New Jersey’s foremost laser vision and corneal transplant surgeons, has successfully restored vision in the right eye of a NJ woman who was afflicted by a severe case of glaucoma.  Dr Daniel Goldberg utilized a rare procedure to restore her sight.</p>
<p>Dr Goldberg had to apply for and receive special clearance from the FDA to perform a rare corrective surgery using a custom designed Aniridia lens implant.  The Aniridia lens is only approved by the FDA for compassionate use, and is reserved for trauma surgeries where damage to the iris is so severe, traditional methods of corrective surgeries can not be considered.   </p>
<p>According to Dr Goldberg, the patient suffered from malignant glaucoma in her right eye, which caused damage to her iris and rendered her legally blind in that eye.  To correct her vision, Goldberg implanted the custom designed Aniridia Lens and performed a corneal transplant.  The surgery was a remarkable success, and the patient has since had vision restored to her right eye. </p>
<p>Dr Goldberg is one of a handful of eye surgeons who are skilled in implanting a custom designed Aniridia Lens.  Goldberg says that most doctors will never use this technique in their practice because of the process involved to gain clearance and the rarity of such severe eye diseases.  As an experienced corneal transplant surgeon, Dr Goldberg has extensive experience in rehabilitating desperately damaged eyes and was confident he could help her.  “Nothing else would have worked for her, and I knew we could greatly improve her vision with this procedure,” said Goldberg. </p>

			<content:encoded><![CDATA[<p>The Middletown Township Lions Club recently recognized renowned cornea, cataract and LASIK eye surgeon, Dr. Daniel B. Goldberg, for performing corneal transplant surgery on a needy Middletown resident who could not afford the procedure.</p>
<p>Dr Daniel Goldberg performed the corneal transplant surgery at a greatly reduced fee, which was paid with funding provided by the Lions Club.  Middletown Lions Club Chairman Al Siano presented Goldberg with an award honoring his gracious contribution. “Our practice at Atlantic Eye Physicians is about helping others .  It is a blessing to all of us to be able to provide the gift of sight,” said Goldberg.</p>

<p>Dr Goldberg’s award recognizes him as one of the best doctors in the field of Ophthalmology. Dr Goldberg is credited as one of the first laser eye surgeons to help bring advanced technologies in the field to the United States. Dr. Daniel Goldberg has a rare dual certification from both the Council for Refractive Surgery Quality Assurance (an independent, nonprofit, patient and consumer health organization that evaluates and verifies the quality of the surgeries provided by select refractive surgeons in the United States), and the ABES( American Board of Eye Surgeons ). Nationwide, there are less than 10 LASIK surgeons with dual certification by ABES and CRSQA.</p>
<p>Dr Goldberg was a member of the elite team of doctors who performed clinical trials of LASIK in its early stages. This research led to FDA approval of LASIK. In 2000, at the ASCRS(American Society of Cataract and Refractive Surgeons) national meeting, the first large scale study of LASIK monovision was presented by Goldberg and the published paper appeared in 2001 in the Journal of Cataract and Refractive Surgery. Further research by Goldberg resulted in another large study of LASIK monovision which was published in 2003—this work compared results in far-sighted eyes as well as near-sighted eyes. Based on this research, Goldberg made presentations at national meetings on “Succeeding with LASIK Monovision” and taught courses at the ASCRS meetings on “Surgical correction of Presbyopia.”</p>

<p>The purpose of the new study is to confirm the effectiveness of this protocol and to expand the awareness amongst eye care professionals of this inexpensive treatment for EKC. </p>
<p> The treatment of EKC using betadine has been widely debated for years, but new research is shedding positive light on the treatment.  The Ocular Surgery News website, <a href="http://www.osnsupersite.com">www.osnsupersite.com</a>, recently interviewed Dr Goldberg about his new study and about using betadine as a treatmentn for EKC.</p>

			<content:encoded><![CDATA[<p>Earlier this year, the FDA approved Mono-vision LASIK with the VISX CustomVueExcimer Laser.  Mono-vision is a strategy to correct presbyopia without having to wear reading glasses.  Presbyopia is the aging process which reduces accommodation (adjusting the focusing power of the eye for near vision and reading).   The approval confirmed what New Jersey LASIK Surgeon, <strong><a title="Dr. Dan Goldberg" href="http://www.goldberg4lasik.com/about-dr-goldberg" target="_self">Dr. Daniel Goldberg</a></strong>, has known for years.</p>
<p>As we age, our ability to change focus for near vision diminishes because the crystalline lens inside the eye becomes less flexible.  If the eye is nearsighted it may still be possible to read without glasses as we get older, but if the eye is not nearsighted or if vision is corrected for distance with glasses, contact lenses or LASIK, then we generally begin to need reading glasses after 40 years of age.</p>
<p>Eyeglasses, bifocals or progressive lenses provide clear focus for both near and distance vision.   With contact lenses there are several options for reading including bifocal contact lenses or mono-vision fitting of contact lenses.  Mono-vision is a strategy to correct both distance and near vision without wearing glasses, and it can be achieved with contact lenses or with Laser Vision Correction such as LASIK . </p>
<p>With mono-vision, the dominant eye is generally corrected for distance vision and the non-dominant eye is corrected for near vision creating 2 eyes with different focus.</p>
<p><img style="float: right; margin-left: 10px; margin-right: 10px;" src="/wp-content/uploads/goldberg.jpg" alt="" width="166" height="206" />Despite nearly 40 years of experience with contact lens mono-vision, Dr. Goldberg recalls that in the 1990’s “when refractive surgery evolved with excimer laser vision correction, such as LASIK, into a safe and accurate surgical alternative to glasses or contact lenses, there was great skepticism about mono-vision amongst both eye surgeons and patients.”  This is because mono-vision breaks our concept of binocular vision with both eyes having similar focusing power.  Patients wondered if mono-vision would cause headaches and dizziness, and eye surgeons doubted the success and viability of surgical mono-vision.</p>
<p>Daniel B Goldberg MD at the <a title="Atlantic Laser Center" href="http://www.lasiksurgery-nj.com/contact-dr-goldberg-today/">Atlantic Laser Vision Center </a>in Little Silver New Jersey was one of the pioneers in <a title="NJ Laser Vision Correction" href="http://www.goldberg4lasik.com/ilasik-prk" target="_self">New Jersey laser vision correction</a>, and developed a successful practice in refractive eye surgery in the 1990’s.  As he recalls: “Most of the research in refractive surgery at that time related to the development of the laser technology. Prior to my clinical research on mono-vision there were very few reports on surgical mono-vision and no reports on LASIK mono-vision at all.” </p>
<p>In Dr. Goldberg’s practice, the patients over 40 years of age were seeking answers about correcting their reading vision without glasses.  In his practice, up to 50% of the patients over 40 chose mono-vision and the level of success and satisfaction was even higher that patients with contact lens mono-vision.</p>
<p>In 2000, at the ASCRS(American Society of Cataract and Refractive Surgeons) national meeting, the first large scale study of LASIK mono-vision was presented by Goldberg and the published paper appeared in 2001 in the Journal of Cataract and Refractive Surgery.  Further research by Goldberg resulted in another large study of LASIK mono-vision which was published in 2003—this work compared results in far-sighted eyes as well as near-sighted eyes.  Based on this research, Goldberg made presentations at national meetings on “Succeeding with LASIK Mono-vision” and taught courses at the ASCRS meetings on “Surgical correction of Presbyopia.”</p>
<p>Since 2003, there have been several additional studies by other experts on LASIK mono-vision which have confirmed the success previously reported by Goldberg.  Several months ago, the FDA approved the use of the VISX CustomVueExcimer Laser for LASIK mono-vision.  The VISX CustomVueexcimer laser is part of the advanced iLASIK surgical platform distributed by American Medical Optics, and CustomVueMono-vision is part of the iLASIK program.  NASA and the US Navy and Air Force have recently chosen this technology for LASIK on their pilots.  The Atlantic Laser Vision Center in Little Silver New Jersey is one of several centers in New Jersey offering iLASIK.</p>
<p>In April, at the inaugural training sesseion for CustomVueMonovison in Chicago, Goldberg was honored to be recognized for helping to set the stage for the FDA approval of LASIK mono-vision.  Goldberg feels that it was an opportunity of a lifetime to make a substantial contribution to his field:  “It is gratifying to see that LASIK mono-vision is now an accepted option for the surgical correction of presbyopia, and the FDA approval will enable more of our patients to safely enjoy greater freedom from glasses.”</p>

			<content:encoded><![CDATA[<p>There are two organizations which provide certification of LASIK surgeons based on surgical skills and results.  Dr. Daniel Goldberg has a rare dual certification from both the <a href="http://www.usaeyes.org/surgeons/daniel-goldberg.htm">Council for Refractive Surgery Quality Assurance</a> (an independent, nonprofit, patient and consumer health organization that evaluates and verifies the quality of the surgeries provided by select refractive surgeons in the United States), and the ABES(<a href="http://www.aces-abes.org/">American Board of Eye Surgeons</a>).</p>
<p>The ABES(American Board of Eye Surgeons) promotes the highest possible quality ophthalmologic surgical care.  The ABES certification programs include a process through which a surgeon’s actual surgical skills and judgment are tested and observed by his or her peers.  The ABES tests surgical skills in cataract/intraocular lens surgery as well as LASIK and corneal transplantation.  In 1989, Dr Goldberg was the first eye surgeon in New Jersey to have surgical skills in cataract and intraocular lens surgery certified by ABES.  In 2002 he became one of 2 New Jersey LASIK surgeons to have surgical skills certified in LASIK by ABES.</p>
<p>The other organization to certify LASIK surgeons is the non-profit, patient advocacy CRSQA(Council of Refractive Surgery Quality Assurance).  According to CRSQA, certification is an important indication of the quality of a LASIK doctor, and once certified, results are re-evaluated every three months.  There are fewer than 250 doctors nationwide who have attained and maintain certification by CRSQA.</p>
<p><strong>Nationwide, there are less than 10 LASIK surgeons with dual certification by ABES and CRSQA</strong>.  Daniel B.Goldberg MD of the Atlantic Laser Vision Center in Little Silver, NJ is the only NJ eye surgeon and one of only 10 eye surgeons nationwide to have dual certification in LASIK based on review of actual surgical skills and results.</p>

<p><a title="Laser Vision Correction" href="http://www.goldberg4lasik.com/ilasik-prk" target="_self">Laser Vision Correction</a>, including LASIK and related procedures such as PRK(photorefractive keratectomy), and ASA(advanced surface ablation) have become the most common refractive surgical procedures with over 1.4 million procedures yearly in the US.  Steady advances in technology have contributed to the quality of results, and the safety of the procedure.</p>

<p>The FDA scrutinizes and approves the safety of laser eye surgery technology.  Dr. Goldberg advises patients to seek the best combination of technology and an experienced surgical team.  He feels that surgical certification of actual surgical results is an important protection for patients:  “It demonstrates that the surgeon has the confidence to have their surgical skills and results evaluated.  I have always supported the principle that surgical skills and results should be evaluated objectively.”</p>

<p>FACTORS INFLUENCING MONOVISION’S SUCCESS</p>
<p>Patients’ Expectations</p>
<p>The goal of monovision is to increase patients’ functional vision for daily tasks without the aid of glasses.<br />
This endpoint, however, does not preclude a person’s ability to use glasses for tasks requiring clear binocular distance vision (ie, night driving) or sharp binocular near vision (ie, prolonged reading). Although some patients are satisfied with monovision, they face disadvantages such as less-than-ideal distance vision; the possible need for glasses when driving at night; two eyes, each focusing differently; and the adjustment involved with this form of correction.1-4</p>
<p>Ocular Dominance and Interocular Blur Suppression</p>
<p>Interocular blur suppression is essential to successful monovision. In each person, the input from one eye or the other is usually dominant in influencing binocular cells in the cerebral cortex. Information from an individual’s dominant eye produces a greater response to a given stimulus than input from his nondominant eye. Correcting the dominant eye for distance and the nondominant eye for near is standard, because (1) correcting the dominant eye for the most commonly used viewing distance maximizes blur suppression and (2) the dominant eye maximizes the performance of visual tasks requiring spatial perception.4</p>
<p>Although this approach is the norm in the studies reviewed for this article, Jain et al 4 reported that 43% of 42 study patients were treated with crossed monovision (wherein the dominant eye is corrected for near), and subjects’ satisfaction ranged from 72% to 86%. Although this range was relatively high, it was the lowest rate of patient satisfaction among the studies reviewed herein.</p>
